PLEASE SUBMIT YOUR 3 YEAR MOTOR VEHICLE REPORT (MVR) ALONG WITH THE APPLICATION FOR CONSIDERATION. Applications received without the MVR will be considered an incomplete submittal. You can obtain this report from your local Department of Driver Services or Department of Motor Vehicles.
